HIGH SCHOOL SOCCER CAMP

Afternoons, 2:30 - 5 p.m.
2:30 - 3:15 pm - Weight Room with Coach Spiewak
3:15 - 5:00 pm - On field with Coach Crowley and Coach Smith (stay in gym shorts to start, bring soccer gear)

First Day of August Practice 

Fall tryouts will be August 12, 13 and 14 from 3-6 p.m. Players should meet in the stadium and come ready to go in running shoes. Bring all of your soccer equipment as well.

** An up-to-date physical must be completed prior to the start of the season.

This camp is geared toward those student-athletes who are interested in trying out for the basketball team in November. The camp will cover expectations, offensive style of play and defensive principles of Notre Dame Basketball. This camp does not guarantee a roster spot on the team.

Time:  The July 9 camp will be held from 12 - 1:30 p.m at Prairie View Courts in Morton Grove. All other dates will take place from 10 - 11:30 a.m. at Prairie View Courts in Morton Grove.

RELEASE AND WAIVER OF LIABILITY

This Release and Waiver of Liability (the “Release”), acknowledged on the date it is accepted by a parent or guardian of any minor child, is for the benefit of Notre Dame College Prep, a not-for-profit organization organized and existing under the laws of the State of Illinois, USA, its directors, officers, employees, and agents (collectively “NDCP”). It is understood, that the failure to acknowledge acceptance of the terms contained herein will result in the applicant being denied the right to participate in the NDCP Summer Athletic Camp. We hereby freely and voluntarily, without duress, execute this Release under the following terms:

1. Waiver and Release. We, on our behalf and on behalf of our child, release and forever discharge and hold harmless NDCP and its successors and assigns from any and all liability, claims and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from our child’s participation in Summer Athletic Camps at NDCP.
2. Insurance. We understand that NDCP does not carry or maintain health, medical, or disability insurance coverage for participants. Insurance is the responsibility of the participant.
3. Medical Treatment. We hereby release and forever discharge NCDCP from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during our child’s participation in NDCP sponsored Athletic Summer Camps.
4. Photographic Release. We grant and convey unto NDCP all right, title, and interest in any and all photographic images and video or audio recordings made by NDCP during our child’s participation in NDCP Summer Athletic Camps, including, but not limited to, any royalties, proceeds, or benefits derived from such photographs or recordings.
5. Other. We expressly agree that this release is intended to be as broad and inclusive as permitted by the laws of the State of Illinois in the United States of America, and that this Release shall be governed by and interpreted in accordance with the laws of the State of Illinois. We agree that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the validity of such clause or provision shall not otherwise affect the remaining provisions of this Release, which shall continued to be enforceable.
6. The individuals acknowledging this agreement and indemnify NDCP from any and all lawsuits including suits filed by third parties against any loss, cost or expense incurred by the NDCP as a result of any action taken by the NDCP with regard to the NDCP Summer Athletic Camps, including, without limitation, all court costs, reasonable attorney’s fees and other costs of collection or defense.

To express our understanding and acceptance of this Release and Waiver, we acknowledge the Agreement.
